We are excited to share that Perth and Kinross are involved in the universal pilot:
Principles into Practice Trial Programme
This pilot seeks to improve the planning delivery of support for young people ages 14 to 25, who require additional support as they make their transition to young adult life.
This work is based on 7 Principles of Good Transition
- Planning and decision making should be carried out in a person centred way
- Support should be coordinated across all services
- Planning should start early and continue to age 25
- All young people should get the support they need
- Young people, parents and carers must have access to the information they need
- Families and carers recognised as needing support
- There needs to be a continued focus on Transition in our area
The first information session in relation to this work has taken place in Perth and Kinross and you can see a video recording of it below. During this session Lee House and Andrew Batchelor share information about the opportunities available through the Independent Living Fund. This fund is available to support young people who require additional support with their transition into young adult life

Resources to support transitions
Together for Short Lives have created a 27 page guide for young people with life threatening conditions making the transition into adult services. The guide - Moving to Adult Services, what to expect. A really useful and comprehensive guide for the young people it is designed for but also for anyone supporting a young person through the transition phase.

The Scottish Transitions Forum have a Parent and Carer Network section within their website where people who are parents and/or carers of young people with additional needs can have a voice. You can access the website here.
